Request for Response to SEMI Survey
Michael Huff
2003-04-18

Dear MEMS Exchange, MEMSNet and MEMS Talk users:

I am sending this message to you on behalf of the SEMI Standards
Working Group on MEMS technology. As you probably know SEMI
(Semiconductor Equipment and Materials International) is the leading
trade organization and standards forming body for the semiconductor
industry. Recently SEMI has initiated activities in support of the
MEMS industry.

We are asking leaders in the community, such as yourself, to complete
a survey questionnaire with the goal of better understanding starting
material requirements for the MEMS industry. We believe that this
information will help MEMS manufacturers and their suppliers to better
understand the issues involved and thereby improve their products.

The survey is at:

      http://www.memsnet.org/survey/semi/

Your opinions on this topic are extremely important to us and we hope
that you will be able to spend the estimated 10-15 minutes of your
time to fill it out and email it back to us. In order to help
encourage people in the community to dedicate the time to this task,
as well as allowing us to demonstrate our sincerest appreciation, the
MEMS Exchange will select one respondent on this survey at random to
receive a credit toward processing services through the MEMS Exchange
in the amount of $500.

This credit for processing with the MEMS Exchange is not transferable,
limited to United States entities, and must be used within the 6
months period after receipt. Limited to one survey response per email
and person. Multiple survey responses from a single person or email
address will be discarded. Eligibility limited only to fully
completed surveys. Survey must be received by 11:59 p.m. EST May 9,
2003 to be eligible for processing credit. To claim credit for
processing, person must register with the MEMS Exchange and set up
business account as well as execute the MEMS Exchange Customer
Agreement without modification.

Please send the responses to [email protected] or to the fax
below. If you fax this message back to me, please provide your name
and contact information for prize notification.

Sincerely,

Michael A. Huff
